Request a Letter

Please use this form to request a letter from the practice. 

The preparation of these letters or reports is not covered by the NHS. The GP must complete them during their own time and as such there is a fee payable. You will be notified of the fee for the type of letter you are requesting. As a minimum, a simple letter will cost £15; the fee may increase up to £55 if it is a more complicated letter or report. 

Once submitted, your request will be reviewed by our administration team and then passed onto the GP. The GP reserves the right to deny a request for a letter depending on the information you provide and you will be notified if that is the case. You may be contacted if the GP requires more information. Please note, private letters do not take priority over NHS work and it may take some time to prepare. Please allow at least two weeks when submitting your request.
