Cedar Practice wants your ideas and opinions

Would you like to help Cedar Practice deliver and develop our patient services?

We are looking for patients from all age groups and social and cultural backgrounds.

We already have an active patient participation group and we would like to expand this and gather the views of a wider and larger representation of our practice population.

To do this we would like to create an email group so we can canvass views without the need for people to attend meetings.

Would you like to be involved?

Please pick up a leaflet on Patient and Public involvement at the reception desk, or view the PDF version below.

We would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we are setting up a virtual patient representation group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance

We want to make sure that local people across City and Hackney have access to the care and support they need. North East London CCG actively seek opportunities to find out what our local communities in our area think and feel about their local NHS and care services through a range of activities including surveys, focus groups and workshops, as well as through partner organisations and patient forums.

You can get involved in a variety of ways:

  • Get involved in NHS Community Voice, a patient led project hosting community conversations and open events group of local residents and patients organising regular events and workshops in the community (email: lola@healthwatchhackney.co.uk); or take part in one of our neighbourhood based projects to help shape the services of a specific locality in City and Hackney (email: sabrina@healthwatchhackney.co.uk)
